Search Tool - Thinking of attempting a New Year's Resolution? Search our website to find help with quitting smoking, weight loss, strength training or anything else you would like to know.

mySFM Login - You can log onto mySFM from any page on the new site.  With mySFM, you can make appointments, get test results, get a prescription refilled, request referrals or contact your doctor. If you send an e-mail to the office, we will answer your questions within 48 hours, usually sooner. We use the latest technology to ensure that the information you send to us and the information we send to you is safe.

Patient Forms - Do you ever get into the doctor’s office and forget all the things you wanted to talk about?  You can fill out a pre-appointment form that is specific to your age group. That way, you won’t forget to ask for that refill or talk about a symptom.

Spend Some Time with our Providers - If you want a new doc or are seeing a provider for the first time, check out our video biographies (bios).  These short videos are a great way to learn more about the provider and their education, experience and interests.  Some of the providers are multi-lingual and many offer specialties, such as sports medicine or obstetrics. These videos can help you decide which provider is the best fit for you and your family.

Careers - Are you looking for employment?  We are often looking for qualified applicants and now that information is available online. Check out our job listings and employment video.

Scholarships for Patients - SFM provides scholarships to high school seniors who are patients or children of patients. Access the scholarship forms online and see what options are available to your high school seniors.
